HHS Uncovers Gender Clinics’ Secret Cash Grab

While the Trump administration has done great work exposing the evils of the gender mutilation mills, the hospitals and doctors performing these procedures have found ways to hide their actions. It is no surprise that the industry that coerces parents and children to undergo extreme mutilative procedures may be engaging in medical billing fraud. A new report published by the Department of Health and Human Services (HHS) titled Wolves in White Coats: How Doctors and Hospitals Pushed and Profited from the Fraud of “Gender Medicine” details their seedy activities.

The report looks at more than 225 pediatric gender clinics nationwide. These clinics have billed a total of $120 million for sex-rejecting surgical procedures and 8,500 courses of puberty blockers or cross-sex hormones for minors from 2015-2025. Approximately $50 million in insurance claims have been coded as an “unspecified endocrine disorder” for children ages 9-17 to disguise prescriptions for puberty blockers. In addition, $11 million in claims were coded for “precocious puberty.” This coding is intended for children who begin puberty at an abnormally young age, but the clinics were using it for kids aged 13-17 exactly when puberty is expected to occur!

Multiple whistleblowers, parents, and patients shared their first-hand testimonies about the lack of safeguards around sex-rejecting procedures. HHS filmed a short-form documentary, published on YouTube, featuring several detransitioners who lived through the gruesome, coercive experience of transitioning as a minor.

One detransitioner featured in the video is Clementine Breen, who began struggling with her body image at age 12. A school guidance counselor suggested she may be experiencing gender dysphoria and reached out to her parents. Within months, the counselor facilitated her social transition and gave lessons on transgenderism to her entire class. After an hour-long appointment at Children’s Hospital in Los Angeles, doctors told Clementine’s parents that she was “100% trans.” Out of fear for their daughter’s mental health, her parents agreed to puberty blockers, testosterone, and eventually a double mastectomy. This was paired with multiple psychiatric drugs.

Despite these invasive procedures, Clementine’s mental health problems escalated to psychotic episodes. After going through therapy for her underlying conditions such as trauma from prior sexual abuse, she realized that her gender transition was not, in fact, the cure for her problems. She began to detransition at age 18. The intense scrutiny she faced from peers and medical professionals about reconstructing her breasts or re-feminizing her face to correct what was done to her did not occur before her mutilation surgeries. She still struggles with long-term symptoms from the puberty blockers, such as chronic joint pain and bone density loss.

Another detransitioner featured in the film is Layla Jane. At 11 years old, Layla began puberty, much earlier than her peers. After spending a lot of time on the internet and finding a post about LGBT identities, she felt that being transgender was “a really plausible way to escape being a woman.” She was seeing a therapist at the time who kept her social transition a secret from her parents and immediately wrote referrals for her to see transgender specialists. She was prescribed birth control pills to subdue her female hormones, given Lupron to stop breast formation and menstruation, and placed on testosterone a year later. At 13 years old, Layla underwent a double mastectomy. However, 4 years later, she began questioning these decisions and detransitioned.

Now, Layla is left with a damaged liver, genital atrophy, urinary issues, and nerve pain. She describes her symptoms as those of a “menopausal woman.” She found out that her mammary glands were still intact, which could cause a serious infection if she were to get pregnant and start lactating without a place for the milk to be expelled. When she read that another detransitioner, Chloe Cole, was suing the same doctor who performed these procedures on her, she contacted lawyers as well. She sued Kaiser Hospital Foundation, seeking punitive damages for malice, oppression, and fraud. While the statute of limitations for her harm has passed, Kaiser has halted all “gender-affirming” surgeries since August 2025.

Clearly, these types of procedures violate the Hippocratic Oath to “do no harm” to patients. HHS is calling for state and federal investigations into potential medical billing fraud for gender transition procedures.  This must happen so that these doctors cannot hide their horrific practices while profiting from insurance and taxpayer money. Some states have already begun. Texas Attorney General Ken Paxton filed a healthcare fraud lawsuit against two physicians who were concealing gender transition procedures. One doctor has surrendered her license, and the other is still ongoing.

Other states need more scrutiny from the federal government for not complying. The state of Pennsylvania alone has footed the bill for $76 million worth of puberty blockers for minors after seeing a 2000% spike in these procedures over the last 15 years. Pennsylvania Governor Josh Shapiro is protecting these clinics. He even submitted an amicus brief in favor of the Children’s Hospital of Pennsylvania’s lawsuit against the Department of Justice, which is investigating their fraud.

Congress has been drawing a lot of attention to the horrors of gender mutilation. This year, the Chloe Cole Act (H.R. 7651) has moved out of the House Judiciary Committee and is headed toward a floor vote. The bill will provide a private right of action for individuals who were transitioned as minors to sue healthcare providers, hospitals, and clinics that participated in these procedures. It also extends the statute of limitations to 25 years past the age of 18. If the Chloe Cole Act is signed into law, fewer facilities and doctors will be willing to take such a risk and will effectively shut down the industry. 

Planned Parenthood is also in the gender mutilation business and has conflated abortion with “gender affirming care” for years. Recently, Planned Parenthood posted “Abortion is gender-affirming care,” leading us to wonder if they are participating in the same medical billing scheme as other gender clinics. Could Planned Parenthood be billing insurance for abortions under medical coding reserved for gender-related procedures? This is why Congress must defund Planned Parenthood immediately.
